Confidentiality Statement
We are committed to maintaining the confidentiality and privacy of all client information. As part of our clinical documentation process, we utilize a secure, third-party Electronic Health Record (EHR) system to store and manage client records.
Use of Third-Party EHR
All client information, including clinical notes, treatment plans, and communications, is documented and stored within a HIPAA-compliant EHR platform.
The EHR provider is contractually obligated to maintain the confidentiality, integrity, and security of all Protected Health Information (PHI) in accordance with federal and state privacy laws.
Client Privacy and Data Security
Access to client records is restricted to authorized personnel only.
All data entered into the EHR is encrypted and protected by secure login credentials and multi-factor authentication.
The therapist ensures that documentation is accurate, timely, and limited to information necessary for treatment and administrative purposes.
Third-Party Responsibilities
The EHR provider does not access or use client data for any purpose other than system maintenance, support, or as required by law.
A Business Associate Agreement (BAA) is in place between the therapist and the EHR provider to ensure compliance with HIPAA regulations.
Client Rights
Clients have the right to request access to their records, request amendments, and receive information about how their data is used and protected.
Any concerns regarding confidentiality or data handling can be addressed by contacting Laura Carpenter, LCSW.
